I understand that a travel fee $15 travel fee will be applied to event locations that are more than 15 miles from our location in Pooler, GA. Locations that are farther than 50 miles from Pooler, GA will be charged $0.575 per mile instead.
I understand that if I have requested for Tummy Time Foods to solidify my event location, an additional $25 fee will apply and is due along with the full facility rental fee at the time of booking.
I understand that a 50% deposit is due to solidify my booking. The balance is due 14 days prior to my event (unless otherwise indicated). A nominal processing fee will be applied to all credit/debit card payments through Authorize.net. Upon submission of this Private Party Booking Request, an invoice will be emailed to you for the event deposit and balance.
If a party is cancelled at least 14 days prior to the party date, a full refund will be issued. A refund will not be issued for cancellations made 14 days or less prior to the event date. If the event date is changed at least 14 days prior to the party date, all deposits and payments will be credited to the new event date. No credit will be given for “no shows”.
Parent/Guardian Consent of Participation and Waiver
I acknowledge that I have read and accepted Tummy Time Foods policies. I hearby release Tummy Time Foods and all its employees, contractors and volunteers from all claims, demands, losses, actions, suits or precedings rising out of the participation in any programs offered in any facility or location where a Tummy Time Foods program is being held. I understand that Tummy Time Foods has tried to create a safe and controlled environment for participation and has established rules for participation in the kitchen area that must be followed by the participant. I also understand that failure to comply with any of the policies of Tummy Time Foods may result in removal from the class or event without a refund. Participation in a Tummy Time Foods class or party may involve student use of cooking equipment and tools in a busy kitchen environment. It is understood and expressly agreed to by the Parent (or student) that by making payment (including the redemption of a gift certificate or volunteering) and/or by participating in the class or event, the Parent/Student releases, indemnifies and holds harmless Tummy Time Foods, its owner(s), independent contractors, employees and volunteers from any and all liability of any kind for any damages and/or injuries incurred in connection with the students’ attendance at a Tummy Time Foods event and understands and accepts the risks inherent in the preparation, cooking and eating of food with other people in the kitchen.
Names, ages and food restrictions/allergies of all guests and names of responsible adults staying for the event are due seven days prior to the event. One responsible adult per child is required for guests ages 5 and under. For groups ages 6+, at least two responsible adults are required to stay for the duration of the party. All participants should wear comfortable clothing, closed toe shoes and pull back long hair. Parents of each guest must sign our Waiver and Release Form prior to the start of the event.
